A non-refundable registration fee of $30 is due at the time of your child's enrollment.

Please print the registration form below and mail with registration fee to Hope School-Age Care. You will need Acrobat Reader to view and print the registration form. Your computer probably has it. If not, you may obtain it for free at Adobe.
